Please apply by sending the application form through postal mail, upon confirming the following points.
Please note that we do not accept any requests from third parties on behalf of the applicant.

<Documents to be Submitted>
[1] In the application, to confirm your personal identity (or that of your legal representative), please attach a copy of your residence certificate or your driver’s license.
[2] Return postage of 800 yen (We will send you return mail by registered mail with a return receipt requested).
If you wish to have your personal information deleted, return postage is not necessary.

<Points of Caution>
[1] The personal information contained in your application form or copy of your residence certificate or driver’s license will be used within the scope of application processes requested by the customer, and will not be used for any other purpose.
[2] The application form and copy of the residence certificate or driver’s license will be discarded within 1 month of the company receiving them.
[3] Our company will not directly contact you by phone, etc., regarding the contents of the personal information written on your application form. In addition, please note that we do not accept customer inquiries by phone.
[4] Regarding the personal information on your application, we will only notify you in writing if the name and address, etc., of the customer applying is an exact match for the data our company has.
If the data does not match, we cannot respond to your information disclosure request, and will send your return postage back to you.
